Support in caring for yourself or a family member with CF

We welcome your request for assistance no matter where you live, your age, or your level of income. To apply, print out the form below and fill it out completely; please don’t leave any questions unanswered. Then scan and email the application form back to us at cfbridgeofhope@gmail.com.

Cost

Treatment through CF Bridge of Hope is provided free of charge and, depending on your financial situation, includes assistance with daily travel to our clinic and housing during your stay, which usually lasts two weeks. We ask you to pay the cost of travel, if possible, from your home country to the US.

We make every effort to provide medication and therapeutic equipment at no charge during your stay, as well as for an extended period once you leave our clinic. Many of our patients benefit from return visits to receive medication, address any concerns, and improve their respiratory and nutritional status.

Travel

CF Bridge of Hope care is provided in Stamford, CT in the US. By law, one caregiver may accompany the individual with CF who travels for care. Both travelers must have valid passports in order to apply for our program. Upon submitting your application for care, both should begin the process of getting VISA applications in order.

Ongoing

During your stay, you will learn about daily treatment plans, proper use of medications and equipment, and nutritional requirements. Once you return home, you will be invited to online workshops that will keep you up to date on best practices in CF care, including a review of the most current therapeutic equipment and treatment.
